GENERAL DATA PROTECTION REGULATION
As of the 25th of May 2018 The Data Protection Act 1988 ( DPA88) has been replaced by the General Data Protection Regulation ( GDPR). In order to find out more about the GDPR and how we process your information, please click on Patient Privacy Notice on this page or ask a member of our staff for a copy. All patients have a right to access their data and any supplementary information held by Jamieson Medical Practice. If you require a copy of any medical information you must complete one of the request forms, for either copies of full medical records or part medical records. Please note Jamieson Medical Practice will deserve the right to refuse any excessive request for copies of medical records. (we feel that requests for more than one copy to be excessive). Please ensure you keep your copy in a safe place and please note it is your responsibility to make any subsequent copies of your records you may wish to give to any other parties. ie. Insurance companies etc. You can request a copy of any information recorded in your records after the date you received your original request. WHO DO YOU WANT TO HAVE ACCESS TO YOUR MEDICAL INFORMATION? If you wish to share your medical information with someone else or wish someone else to be able to contact the surgery on your behalf we must have your written consent.
